What to Expect When Death Occurs

Whether a death is sudden or expected, the loss of a loved one is indescribable. When you are in a heightened emotional state, even the most basic decisions can seem staggering.

If Death Occurs at Home

The death must first be pronounced by someone with authority to do so (Hospice nurse, doctor, sheriff, paramedic or coroner). If none of the above is present to pronounce death, family should:

Call 911. Inform the operator that a death has occurred.

If the death was expected, you may want to suggest to the operator that no sirens or lights are necessary.

Request an authority to pronounce the death.

A family member or nurse should call Peaceful Reflections at (661) 251-0500 and advise us that a death has occurred. We will make arrangements to bring your loved one into our care and transport to our Care Facility.

On the next business day Peaceful Reflections will contact the family or authorized agent to set up appointment to discuss arrangements and sign authorization documents (unless previous arrangements have been made).

If Death Occurs at a Hospital, Hospice, Nursing Home or Health Care Center

The death must first be pronounced by someone with authority to do so. Then a family member or the facility calls Peaceful Reflections at (661) 251-0500 and advises us that a death has occurred.

We will make arrangements to bring your loved one into our care and transport to our Care Facility.

On the next business day Peaceful Reflections will contact the family or authorized agent to set up appointment to discuss arrangements and sign authorization documents (unless previous arrangements have been made).

If Death has Occurred and the Medical Examiner or County Coroner is involved

Call Peaceful Reflections at (661) 251-0500.

Peaceful Reflections will set up appointment with family or authorized agent to discuss arrangements and sign authorization documents.

We will need a Coroner’s release form signed by the Authorized next of kin. Once the investigator clears a case and the Coroner’s office advises us that an individual is ready for pick-up we will make arrangements to bring your loved one into our care.

Most Coroner’s have a fee. In Los Angeles County, the Coroner will bill you directly for their service fees.

Peaceful Reflections will receive your loved one into our care and transport to our Cremation Care Facility in Canyon Country.

Once Authorizations have been signed and your loved one has been brought into our care

Peaceful Reflections will prepare the death certificate for the attending physician/coroner to complete and sign. We will work with the county Health Department and the physician/coroner for the signature, and file the death certificate with the county and state Health Department.

In addition we will process and file the appropriate documentation with the State and County Registrar departments for the cremation/disposition permit. Cremation Permit from the County Health Department must be issued prior to the scheduling of cremation.

Cremation can be scheduled after the cremation permit is issued. Cremation typically occurs within 7-10 working days (dependent upon all documentation being signed by legal next of kin/authorized agent).

Disposition of ashes takes place, i.e., release of loved ones cremated remains to family, scattering at sea or delivery to cemetery for placement.

Burial can take place after the death certificate and burial permit have been filed with the County Registrar and cremation has taken place. Arrangements must be made with the cemetery to receive your loved one.

We will process and obtain your initial request for Certified copies of the Certificate of Death with the health department.
